Notes:

  1. Please enter one product type and description of fault at a time, if you have more than two products you will need to submit this form again.
  2. All fixed products will be returned to the site address provided. If you require us to return the fixed products to an alternative address, please complete the 'alternative address' information.
  3. All returns require a customer order number and will not be accepted unless a valid number is provided. The customer order number will be used as a reference upon the invoice.
  4. Where a piece of equipment is returned as faulty but is still in warranty, it will be subject to examination for user damage and liquid ingress, and if accepted will be repaired under the warranty terms free of charge.
  5. By submitting this form you acknowledge that you have agreed to these terms and conditions.
  6. Anyone wishing to return a product for repair must have a valid and up to date account as failure to comply will mean the repair cannot be processed.
